ACTIVE MEDICAL STAFF

Members:

  • Duly licensed physicians to practice medicine in the Philippines
  • Active members in good standing of the Davao Medical Society, Philippine Medical Association and their specialty or subspecialty societies.

Classification of Members:

  • Active Rotating Consultant: specialists or subspecialists who are given the privilege to attend house cases of the hospital on rotation basis.
  • Active Associate Consultant: specialists or subspecialists who in special occasion when all active members of the department are not available, maybe allowed to handle house cases on rotation basis.
  • No Active Rotating or Associate Consultant can be classified under more than on Specialty or Subspecialty.

Qualifications for Membership:

  • Diplomate or Fellow in their specialty or subspecialty societies
  • Must be a Visiting Consultant for at least one year.
  • Graduates of accredited Subspecialty Fellowship who are Non-Diplomate or Non–Fellow in their Subspecialty Societies maybe appointed as Active Rotating Consultant temporarily for two years, if the subspecialty is not available in the Active Staff. He must take and pass the subspecialty examination before his re-appointment.

Application to the Active Medical Staff must be addressed to the Medical Director with the following credentials (Xerox copy)       

  • Medical School Diploma
  • PRC Registration Certificate
  • Certificate of Residency Training and/or Subspecialty Fellowship
  • Certificate as Diplomate or Fellow of the Specialty and/or Subspecialty
VISITING MEDICAL STAFF

Members:

  • Duly licensed physicians to practice medicine in the Philippines
  • Active members in good standing of the Davao Medical Society, Philippine Medical Association and their specialty or subspecialty societies.

  • Diplomate or Fellow of their Specialty Society upon application or within the next two years after the approval of the application.

    3. Certificate of Residency Training and/or Subspecialty Fellowship
    4. Certificate as Diplomate or Fellow of their Specialty or Subspecialty Society
    1. Philhealth Accreditation Card
    2. TIN Number

Procedure for Application:

  • Written application with necessary credential addressed to the Medical Director.
  • Upon filing, Medical Director will issue temporary Visiting Privilege for ninety (90) days provided that the required credentials are complete.
  • Medical Director forwards application with credentials to Credentials Committee.
  • Credentials Committee will evaluate the submitted credentials, if complete, will forward application to concerned Department.      
  • Department evaluates the credentials and makes recommendation for approval or rejection and returns application to Credentials Committee.
  • Credentials Committee will present application with complete credentials and department recommendation to the Medical Executive Council for approval of the department recommendation.
  • Medical Director will present the application to the Board of Trustees for appropriate action.
  • Hospital Administration will inform in writing the application of the result of the application after being acted upon by the Board of Trustees of San Pedro Hospital of Davao City, Inc.
OFFICE CLINICS:
  • Application for Office Clinic space must be addressed to the Hospital Administrator.
  • Only Active Medical Staff can hold office clinic in the Main Building.
  • Priorities in holding office clinics in the Medical Arts Building.
    1. Members of the Active Medical Staff
    2. Sub-specialists of Subspecialty that is not available in the Active Medical Staff
    3. Sub-specialists of Subspecialty already available in the Active Medical Staff but not to exceed four.
  • Office Clinic occupants are not allowed to sub-lease the space without the prior approval of the Hospital administrator.
  • Office clinic occupants who will vacate the office clinic space must return it to the Hospital Administrator. It should not be endorsed to other doctors.
  • Rentals must be paid to the Hospital Cashier at the end of the month. Failure to pay for two months, a reminder will be sent. Failure to pay for four months means eviction.
  • Any violations with these policies maens eviction of the occupants.
  • The Hospital Administrator should coordinate with the President of San Pedro College that only Sub-specialists are allowed to hold clinic in San Pedro College Doctor’s Clinic.
